SecureHorizons Agent Licensing:

Under the SecureHorizons contract an agent can sell branded products.

Products:

SecureHorizons has a Special Needs Plan.

• Arizona Physicians IPA – Arizona
• Great Lakes Health Plan – Michigan
• Americhoice – New Jersey, New York and Tennessee
• Evercare – state availability will need to be verified

Contracting:

To become contracted with SecureHorizons, agents need to complete the following:

• Appointment Application
• Agent Agreement (the agent would only need to send to UIG pages 1 and 15)
• W-9 form
• EFT (if interested in having their commissions directly deposited into their checking account)

Agents also need to include a copy of their agent license and E&O insurance. After the information has been received, agents will be issued a temporary identification number, along with instructions on beginning the certification process.

Supplies:

Once contracted, agents can order supplies from www.unitedhealthproducers.com.
